Of course I selected natural for my couple's first birth (And may do a c-section a 2nd time around). So pretty much once your sim hits the 2nd trimester, you'll have to call up your obstetrician and either select a natural delivery, C-Section, or vanilla (aka classic Sims) birth.
And so far, I'm pretty impressed with the mod.
